1. ARRIVAL INSTRUCTIONS and an NRQi Studio PHONE NUMBER will be included in your APPOINTMENT CONFIRMATION EMAIL. Please ensure you read through these prior to your appointment.

2. You are responsible for filling out and submitting the INTAKE FORM prior to your INITIAL IN-PERSON APPOINTMENT or first FACIAL ENHANCEMENT ACUPUNCTURE APPOINTMENT which can be found in your PATIENT PORTAL on the booking site. If the form is not completed and submitted prior to your appointment, then a portion of your treatment time will have to be used to complete the form, as I cannot treat you without your written consent and medical history.
